Project Lay Summary
This project, as part of work package 1, will focus on the barriers and enablers to the implementation of novel capsule sponge technology within primary care. It will look at general practitioner views on current management of gastroesophageal reflux disease and referral processes in both Northern Ireland and the Republic of Ireland. I aim to explore GP perspectives using semi structured interviews to ascertain any concerns or difficulties in current management and whether the use of capsule sponge in primary care could improve pathways in general practice for earlier diagnosis of Barrett’s oesophagus or oesophageal cancer.
Scientific Overview
Research question
Exploring barriers and enablers to the implementation of novel capsule sponge technology within primary care: We will explore the potential barriers and enablers for the implementation of novel capsule sponge technology in primary care on the island of Ireland, through qualitative research, informed by the AllCaN Oesophageal PPI panel members, as part of a supported MPhil project supervised by Professor Helen Coleman and Dr Helen Reid (see collaborator details for relevant expertise). Capsule sponge tests are currently being used in secondary care settings in England and Scotland, as part of an implementation pilot study, to identify suspected Barrett’s oesophagus in those patients with recurrent reflux who would otherwise be referred for endoscopy. They are also used as an adjunct to endoscopy in Barrett’s surveillance programmes. There are also ongoing trials (BEST4) for capsule sponge screening in reflux patients within a primary care setting, with a view to investigating the impact on oesophageal cancer mortality outcomes. However, to our knowledge, there has not been any research to date regarding general practitioner views on the possible use of this technology within primary care. This project will aim to explore general practitioner barriers and enablers on the potential use of capsule sponges in primary care, and how this may affect their current management pathways.
Anticipated impact of this AllCaN Oesophageal research project
Support for early career researcher development will be realised through the MPhil, Dr Órla Carney. Providing information that would aid the potential implementation of capsule sponge technology within primary care, thereby potentially enabling earlier diagnosis for patients with Barrett’s oesophagus and/or oesophageal cancer.
